3x4 Genetics Consultation includes a 30 minute intake appointment (in person) prior to the test and an 60-minute follow-up (in person or video) to review the testing results. Once consultation fee has been paid and initial appointment scheduled, lab test will be ordered and mailed directly to your home. Currently, the turnaround time for results is between 4-5 weeks.

TRADITIONAL MEDICINE
This 60-minute visit is great for the individual (18-64 years old) who has STABLE chronic conditions and is due for their annual visit, routine labs***, and 90-day medication refills, but is having challenges in accessing their primary care team. This appointment fills the gap between your and your primary care team.
This appointment does NOT replace your primary care team. Dr. Hopen does NOT assume the role as your PCP.
It is very important to note that Dr. Hopen does not work directly with any insurance companies as an in-network provider. If you want to use out-of-network benefits, you will be provided with the paperwork you’ll need to submit to your insurer for potential reimbursement. Please contact your insurance company to determine your coverage.
This appointment does NOT include specialty referrals, radiology referrals, ordering any medications requiring prior authorizations. As an out of network provider, Dr. Hopen does not have this capacity to submit referral or address prior authorization paperwork. Additionally, this appointment does NOT include functional medicine consultation.
***Routine lab testing may or may not be covered by your health insurance. The client is responsible for the cost of lab fees. You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services upon request or when scheduling such items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Continuous holistic care is a great option for the individual (18-64 years old) with a high deductible health insurance plan who is having challenges with accessing their primary care team and they want peace of mind knowing that can have timely access to a physician who already knows your history.
This membership fills the gap between you and your primary care team.
This membership does NOT replace your primary care team, and Dr. Hopen NOT assume care as your primary care physician.
So how does this benefit you? If you can't get a hold of your primary care team, instead of waiting in the Urgent Care or walk-in clinic, under this membership, most acute, non-urgent conditions (for example, UTI, sinus infections, vaginal symptoms, allergies, etc.) will be addressed in a timely fashion by Dr. Hopen and in a holistic framework. In addition, if you have any concerns about your chronic conditions, Dr. Hopen can help you figure out the best next steps.
It is important to understand that this membership is limited as Dr. Hopen does not work directly with insurance companies. This membership does NOT include specialty referrals, radiology referrals, functional medicine consultation, ordering any medications requiring prior authorizations.
If you want to use out-of-network benefits, you will be provided with a superbill and you are responsible to submit the paperwork to your insurer for potential reimbursement. Please contact your insurance company to determine your coverage.
